He is portrayed as having multiple arms and heads, often ranging from four to sixteen arms and three to five heads. Each head may have different expressions, including anger, serenity, or fierceness. His complexion is dark or sometimes even black, symbolizing his association with darkness and transcendence.

Bhairav Mahakala is depicted wearing a garland made of human skulls, representing the impermanence of life and the cycle of death and rebirth.
